Early on, it's akin to the dynamics of a newly opened table in an offline setting. It is advisable to adopt a loose-aggressive (LAG) playstyle initially. The reasoning behind this is that gaining an advantage in position early on allows for convenient maneuvering with a wider range. Let's illustrate this with an example.

Consider a 6-handed table with blinds at 200/400/400. In the cutoff position, we have a stack of 20,000 chips and are dealt Ac 5d. Choosing to open with a raise to 1,000, the small blind folds, and the big blind, the initial aggressor, raises to 2,500.

Due to our positional advantage with the medium stack poker strategy we are using, we can opt to call. With an obstructing card Ac in hand, we can occasionally include a 4-bet bluff to balance our range, considering hands like AA or KK. The key consideration is the pot-to-stack ratio post-call and the opponent's range. If we call, the pot will be 5,600, and both players will have 17,500 chips behind. This situation may lead to playing down to the turn card, and our opponent's range is narrowed to 3 combinations of AA, 4 combinations of AK, and 4 combinations of AQ. With only 12 combinations of AA and KK left, leading 16 combinations of AK, we have significant fold equity. Consequently, it's a valid play to push all-in with a 4-bet bluff.

In offline cash games, standard stack sizes typically range from 100 to 200 big blinds, leaning towards medium stacks. Many recreational players and sharks prefer buying in with stacks of 500 big blinds or more for various reasons. Sharks find it convenient for manipulating opponents, while recreational players may simply want to showcase their wealth.

In standard stack cash games, the approach tends to align with ICM strategy close to the bubble. It requires waiting for hands with relatively high starting win rates.

The critical factor for medium stack poker strategy is that our stack provides some hidden implied odds against short stacks. If we rashly call from early or middle positions, we may easily become part of the denominator that reduces effective implied odds. Therefore, adopting a tight-aggressive (TAG) playstyle is recommended. Let's consider an example.

In a 10-handed table with blinds at 20/40, UTG raises to 120, and we are in a middle position with 10s 10h, holding 8,000 chips. It is nearly 100% recommended to raise, isolating players behind us. Although facing a UTG raise with 10/10 is on the edge of being 4-bet, avoiding isolation in this scenario leaves us vulnerable in multi-way pots with 10/10. Therefore, a 3-bet is 100% recommended.

We 3-bet to 720, and the action folds to UTG, who decides to call. The pot is now 1,500. The flop comes Kd/6c/3h.

This flop is suitable for us because the opponent's position is limited, and they likely hold hands like AK and KQ, with some pairs like 8/8-A/A. Since there was no 4-bet pre-flop, the opponent has a range that might include these pairs. Our range has all combinations of Ks, giving us an advantage over hands smaller than A/A. Therefore, it's suitable to continue with a half-pot bet.
